For thousands of years, people saw yellow eyes, dark urine and unexplained liver disease, but no one knew what caused them. Ancient Chinese writings described outbreaks of jaundice nearly 5,000 years ago. Babylonian records also mention similar illnesses more than 2,500 years ago. Yet for centuries, hepatitis remained one of medicine’s greatest mysteries.Doctors could recognise the disease, but they had no idea what was spreading it. Some blamed bad air, others suspected contaminated food or water. The real answer, that different viruses were responsible, would only emerge in the twentieth century after decades of painstaking research.
A strange outbreak after a smallpox vaccination changed everything
One of the most important clues appeared in 1885 at the Bremen shipyard in Germany. After workers received vaccinations against smallpox, 191 people developed what was then called “catarrhal jaundice.” The vaccination itself was not causing hepatitis in the way people first feared. Instead, researchers later realised that contamination during the vaccination process, common before modern sterile practices, had likely transmitted an infectious agent through human lymph used in vaccines.At the time, doctors had no laboratory tests, no microscopes capable of identifying viruses and no understanding of blood-borne infections. All they had was a puzzling observation: people who had undergone the same medical procedure became ill with jaundice.That outbreak became one of the earliest documented pieces of evidence suggesting hepatitis could spread through contaminated blood or biological material. It would take decades before anyone understood what had really happened.
Wars turned hepatitis into a global medical challenge
The mystery did not end in Germany. Large wars repeatedly exposed how easily hepatitis could spread. During the American Civil War, around 40,000 Union soldiers reportedly developed hepatitis-like illness. The problem became even larger during the twentieth century.In 1942, more than 28,000 soldiers developed hepatitis after receiving the yellow fever vaccine. Investigations later showed that some vaccine batches had been stabilised with human serum, unknowingly transmitting what is now recognised as hepatitis B.Then came the Second World War.Historians estimate that nearly 16 million cases of hepatitis occurred during the conflict, making it one of the largest infectious disease burdens of the war. Doctors noticed that some patients became sick after contaminated injections or blood exposure, while others developed illness after poor sanitation and contaminated food or water.These observations slowly revealed an important truth: hepatitis was not one disease. Different infections were behaving in different ways.
The discovery that finally solved the puzzle
In 1965, American physician and geneticist Baruch S. Blumberg was studying inherited blood proteins when he discovered an unusual marker in the blood of an Australian Aboriginal patient. It became known as the “Australia antigen.”At first, nobody realised its importance.Blumberg’s discovery transformed medicine. Scientists could finally develop blood tests to detect hepatitis B, making blood transfusions much safer. It also paved the way for the first hepatitis B vaccine, one of the earliest vaccines capable of preventing a human cancer because chronic hepatitis B is a major cause of liver cancer.For this landmark discovery, Blumberg received the 1976 Nobel Prize in Physiology or Medicine. His work also inspired scientists to search for other hepatitis viruses.Over the following decades, researchers identified hepatitis A, hepatitis D, hepatitis E, and finally hepatitis C in 1989, ending one of medicine’s longest infectious disease investigations.According to the World Health Organization (WHO), viral hepatitis causes around 1.3 million deaths every year, largely due to cirrhosis and liver cancer. Many infected people have no symptoms for years, allowing the disease to silently damage the liver before it is diagnosed.These organisations stress that hepatitis B can be prevented through vaccination, hepatitis C can now often be cured with modern antiviral medicines, and early diagnosis remains one of the most powerful tools for preventing severe liver disease.Each generation of researchers solved one piece of the puzzle without knowing what the next answer would be.Today, millions of people benefit from safer blood transfusions, effective vaccines, accurate laboratory tests and life-saving treatments because scientists kept asking questions about a disease that remained unexplained for centuries.That is perhaps the real legacy of the hepatitis story: medicine moves forward not simply because new technology appears, but because someone notices an unusual event and refuses to stop searching for the answer.
