A tiny diamondback terrapin has become an unexpected discovery on Cape Cod after volunteers found a hatchling with two heads at a protected nesting site in Wellfleet, Massachusetts. According to Mass Audubon’s Wellfleet Bay Wildlife Sanctuary, the unusual hatchling was one of the surprises during the sanctuary’s terrapin nesting season and was taken to a licensed wildlife rehabilitation facility for care and monitoring. The 5.2-gram turtle has a rare developmental condition called bicephaly, according to the Cape Wildlife Center. Veterinarians are now watching its appetite, growth, behaviour and development while they learn more about how its two heads and body function together.

An unusual discovery during nesting season

The two-headed turtle was found during the annual diamondback terrapin conservation work at Wellfleet Bay. For years, volunteers have spent the nesting season searching for terrapins, locating their nests and protecting the eggs from predators. The work is particularly important on Cape Cod because the diamondback terrapin is listed as threatened in Massachusetts. The species lives in brackish environments, where salt water and fresh water meet, and females come onto land during the summer to lay their eggs in sandy areas above the high-tide line.Mass Audubon’s Wellfleet Bay programme has been protecting terrapin nests since 2005. During the 2024 nesting season alone, more than 125 volunteers worked across nine locations in Wellfleet, Eastham and Orleans, protecting more than 200 nests on sanctuary property. The unusual two-headed hatchling was discovered during this wider conservation effort.

The tiny turtle weighs just 5.2 grams

The hatchling is remarkably small. When it arrived at the Cape Wildlife Center in Barnstable on September 17, 2026, it weighed 5.2 grams, roughly the weight of a nickel. Despite its unusual appearance, the turtle has been showing encouraging signs. The veterinary team reported that both heads are alert and that the hatchling has been eating and behaving normally.The condition responsible for its unusual appearance is called bicephaly. It is a rare developmental abnormality in which an embryo develops with two heads rather than separating completely into two individuals. The exact internal anatomy of this particular turtle is still unknown. As it grows, veterinarians expect to be able to use diagnostic imaging such as X-rays and CT scans to better understand how its internal organs and body structures are arranged. For now, the priority is much simpler: keep the tiny animal healthy and watch how it develops.

A rare case even for experienced wildlife rescuers

Two-headed animals are extremely unusual, but this is not the first time wildlife specialists on Cape Cod have encountered one. Mass Audubon’s Wellfleet Bay Wildlife Sanctuary described the discovery of a two-headed diamondback terrapin as one of only a few such cases recorded during the programme’s nearly five-decade history. The Cape Wildlife Center has also cared for other bicephalic terrapins in recent years. That experience means the current hatchling is being watched by a team that has encountered similar developmental abnormalities before. Still, every animal can be different, and veterinarians are waiting to learn how this particular turtle develops. The centre has said that cases like this provide an opportunity to learn more about the anatomy and physiology of these unusual animals.

Why protecting these turtles matters

The discovery comes against the backdrop of a much larger conservation effort. Diamondback terrapins spend most of their lives in narrow strips of salt marsh, while females move onto land to nest. Their eggs and hatchlings face threats from predators including foxes, raccoons and skunks. Roads are another danger, particularly for adult females crossing to nesting areas. Habitat loss, coastal development and the wildlife pet trade also put pressure on populations.Climate change adds another challenge. Mass Audubon’s Wellfleet Bay Wildlife Sanctuary has noted that rising sea levels and unusually high tides can alter traditional nesting areas. During the 2024 season, for example, winter storms pushed a high-tide line farther into one of the sanctuary’s traditional nesting areas, prompting more females to look for alternative sites. Volunteers therefore play a major role in helping the turtles reach adulthood. They patrol nesting areas, look for tracks and nests, install protective cages and monitor hatchlings. In 2024, the sanctuary recorded 4,810 terrapin hatchlings released after 82% of eggs in protected nests survived.

What happens to the two-headed terrapin now?

For the unusual hatchling, the next stage is simply continued observation. Veterinarians are monitoring its appetite, growth, behaviour and development while they learn more about how its two heads and body function together. Its sex cannot yet be determined, and its long-term future remains uncertain. If the turtle grows strong enough to survive independently, wildlife experts could eventually consider whether it can return to the wild. If its condition prevents that, it may instead remain in human care. For now, the tiny terrapin represents both an unusual scientific opportunity and another reminder of the work involved in protecting Cape Cod’s threatened turtle population.What began as another nesting-season patrol ended with volunteers finding an animal few people ever get to see: a 5.2-gram diamondback terrapin with two heads. While its future is still unknown, the fact that it was found, rescued and placed under veterinary care gives the hatchling a chance to grow while experts learn more about this exceptionally rare condition.
