3-year-old girl dies after persistent cough turns out to be fatal heart condition; doctor explains when parents should worry about more than asthma

A cough in a young child is often linked to a cold, allergies or asthma. But when it persists, returns repeatedly or comes with unusual tiredness and breathlessness, it deserves closer attention. The tragic death of three-year-old Penny Dunn in England has renewed questions about recognising less obvious signs of serious illness in children.Penny’s mother, Courtney Sherwood, is now urging parents to trust their instincts when something about their child’s health does not seem right.

Penny Dunn’s cough did not go away

According to Kennedy News and Media, Penny developed a cough around Christmas 2025. The three-year-old, who lived in Gloucestershire, England, had reportedly been healthy and rarely ill before then.Her mother, 27-year-old Courtney Sherwood, said the cough briefly improved before returning and becoming persistent. She and Penny’s father took her to the doctor three times. The family was initially told that Penny had a childhood bug, while asthma was also considered.The cough eventually became severe enough to make Penny vomit. Sherwood also noticed that her daughter became unusually breathless, including after climbing the stairs to the doctor’s office. Inhalers appeared to help for a time, according to the mother.But Penny’s health continued to decline. Sherwood said her once-active daughter became lethargic, refused food and appeared to be losing weight.

Seizure led to the discovery of a serious heart problem

On June 29, Penny suffered a seizure while taking part in a sports day at nursery school. Doctors found that her heart was enlarged and beating abnormally. It was also struggling to pump blood effectively.Penny was transferred to another hospital after medication failed to stabilise her condition. She was placed on advanced life support and diagnosed with dilated cardiomyopathy (DCM).DCM causes the heart’s chambers to enlarge, while the heart muscle becomes stretched and weaker. This can reduce the heart’s ability to pump blood and may lead to heart failure or abnormal heart rhythms.Penny also underwent surgery to address the abnormal rhythm. Her family initially hoped she would recover, but further complications followed. CT scans showed that parts of her brain and bowel had died after losing adequate blood supply.Doctors told her parents that she would not have a meaningful quality of life if she survived. The family eventually decided to withdraw life support. Penny died on July 6.Sherwood said she did not blame the medical team and described the doctors as phenomenal. She also said the cause of Penny’s illness remained unclear, although doctors told the family that a cold might have triggered it.

When should a cough raise concern about the heart?

Dr Manish Bansal, Vice Chairman, Cardiac Care, Clinical & Preventive Cardiology, Medanta Gurugram, said parents should not automatically assume that a persistent cough is a respiratory problem.“I would caution parents against assuming that a persistent cough is always a respiratory problem. In most cases, cough in children is caused by infections, allergies or asthma, but when it is prolonged, recurrent or accompanied by breathlessness, unusual fatigue, poor exercise tolerance, swelling, excessive sweating or difficulty feeding, it deserves a closer medical evaluation.”He explained that some heart conditions can increase pressure in the lungs or cause fluid congestion when the heart cannot pump blood efficiently. This can sometimes produce symptoms that resemble a respiratory illness.However, a cough alone does not mean a child has heart disease. The important concern is a persistent or unexplained pattern, especially when the child’s energy, growth or usual activity changes.

What parents should know about testing

Dr Bansal said a clinical examination can help doctors decide whether further heart investigations are needed, “A thorough clinical examination, followed when indicated by investigations such as an echocardiogram or ECG, can help identify whether there is an underlying cardiac issue.”An ECG records the heart’s electrical activity, while an echocardiogram uses sound waves to examine the heart’s structure and pumping function.Dr Bansal stressed that awareness should not become panic, “The key is awareness rather than alarm. Parents know their child’s normal behaviour best, and any significant or unexplained change should prompt timely consultation with a doctor.”For Sherwood, that message carries a deeply personal meaning.
