Statins and dementia: Cholesterol-lowering drugs may cut dementia risk by 15%, study finds
Starting statins early after type 2 diabetes diagnosis may cut dementia risk by 15%, study finds.

Many of us are already well aware of statins being everywhere in the world of heart health. But it turns out their story might not end with arteries and cholesterol. A major Danish study just shed some fresh light on these common drugs. People who began taking statins shortly after learning they had type 2 diabetes seemed to have a smaller chance of developing dementia over the next decade compared to those who skipped or delayed the pills.The researchers published the study in The Lancet Regional Health – Europe and talked about it at the European Society of Cardiology Congress 2026. Now, here’s why this matters: both type 2 diabetes and high LDL (“bad”) cholesterol are well-known troublemakers. They don’t just raise the risk of heart attacks or strokes; they’re also linked to worsening thinking and memory as people get older.According to this study, starting statins within that crucial first year after a diabetes diagnosis led to a 15% dip in relative risk for dementia over 10 years. Even people who started later (within five years) saw a 10% reduction.But before anyone starts celebrating, let’s be clear: this research doesn’t prove statins prevent dementia. The scientists watched what happened to people over time, rather than giving everyone the same treatment and watching for differences. In the research world, that means you can find patterns and associations, but you can’t say statins are the smoking gun that keeps dementia at bay.

What did they actually look at, and what did they learn?

For starters, the study tracked over 132,000 people. This wasn’t a tiny trial; they dug into the health records of 132,585 people in Denmark who developed type 2 diabetes from 2006 to 2019. None had taken statins before. Researchers split everyone into three groups: some never started statins during the first five years, others began within a year, and the rest started between one and five years after diagnosis. Then they followed each person until they either developed dementia, died, moved, or until the calendar hit the end of 2021.Here’s what happened:Over a typical follow-up period of about seven years, just under 3% (3,592 people) ended up with dementia.Crunching the numbers over a 10-year stretch, people who didn’t start statins saw nearly a 4% risk of developing dementia (3.9% to be precise). Early starters had a risk of 3.3%, while those who began later saw 3.5%.So, in relative terms? That’s a 15% reduction for those who got statins right away, and 10% for those who waited a bit.

Why might statins help the brain, and not just the heart?

Let’s zoom out a bit. Statins are already best known for lowering LDL cholesterol, which creates fatty deposits in blood vessels. Blocked or narrowed vessels can eventually mean heart attacks or strokes. But brains need healthy blood flow, too.Things get more complicated in the brain. Diabetes itself is a red flag for cognitive problems. And when blood vessels throughout the body get damaged, that harm may contribute to types of dementia, especially vascular dementia, and make things worse for brains already at risk for Alzheimer’s disease. The researchers suspect that lowering LDL with statins early in the diabetes journey might protect those tiny, vulnerable vessels inside the brain.Now, this is a theory. While it fits what we know so far, the new data doesn’t prove that’s the real explanation for the dementia drop.

The timing seems to make the most difference

The crucial bit one needs to pay attention to? Timing matters. According to this study, taking statins within the first year after a type 2 diabetes diagnosis makes a bigger difference for dementia risk than waiting a few years. The relative benefits break down like this: a 15% risk reduction with early statins, 10% if treatment starts later on.In real terms, the head start isn’t dramatic. If you look at 100 people, early statin takers avoided about half a case of dementia, or 0.59 percentage points, over 10 years compared to non-users. Those who delayed statins saw a smaller benefit (about 0.38 points lower risk compared to non-users).Notably, both men and women seemed to benefit in roughly the same way.

Most people with diabetes are already prescribed statins for their heart

Don’t count on doctors changing course or prescribing statins just to ward off dementia. People with diabetes are already high on the list to receive statins, simply because they face a much bigger risk of heart trouble. The American Diabetes Association recommends that pretty much everyone with diabetes between 40 and 75 should be on at least a moderate dose of statins, unless they already have serious heart disease.The main reason will always be to protect the heart and blood vessels: a reduction in heart attacks, strokes, and complications tied to high cholesterol. Any dementia-related benefit is still being explored and isn’t the leading reason to take statins right now.

Do statins really stop dementia?

It’s not a slam dunk yet. The study stopped short of claiming statins directly ward off dementia. People who choose to start early might have healthier habits, easier access to healthcare, or be more proactive about their health than those who don’t. There’s just no way to perfectly control for all those differences in an observational study.We need randomized clinical trials, which are the gold standard for proving cause and effect, before anyone can say for sure whether statins protect the brain. The authors and the European Society of Cardiology both urge caution: don’t read this as definitive proof.But the evidence does fit into a larger story. Look across other studies, and you’ll find a similar suggestion: healthy blood vessels support a healthy brain. Past research has even hinted at links between statin use and a lower risk for Alzheimer’s, but the results swing back and forth depending on the study.

What’s the bottom line?

If you have type 2 diabetes and your healthcare provider suggests a statin for heart health, don’t wait around. Start it promptly, and you may be giving your brain a bonus, too. But hold off on taking statins solely to prevent dementia. Talk it over with your doctor, stick to the tried-and-true reasons for cholesterol-lowering medication, and pay attention as researchers keep digging.
