That is exactly what has happened after Gujarat reported a fresh outbreak of Chandipura virus-linked acute encephalitis, with 22 suspected deaths and dozens of infections under investigation, according to state health authorities. The virus is not new. It has appeared in India before, particularly in parts of Gujarat, Maharashtra, Andhra Pradesh and neighbouring states. Yet, every outbreak reminds doctors of one worrying fact: this infection can move alarmingly fast, especially in children.Unlike many viral illnesses that take several days to worsen, Chandipura virus can sometimes affect the brain within just one or two days after symptoms begin. That narrow window makes awareness just as important as treatment. So, what exactly is this virus? Why does it affect children more severely? Can it spread from one person to another? Most importantly, how can families protect themselves?

A virus that most people have never heard of, but doctors have

The Chandipura virus belongs to the Rhabdoviridae family, the same broad family that also includes the rabies virus, although the two diseases are completely different. The virus was first identified in 1965 in Chandipura village in Maharashtra, from where it gets its name. Since then, India has witnessed occasional outbreaks, almost always involving children.Investigation from ICMR and NIV show that the infection has repeatedly appeared in central and western India, particularly during the monsoon and post-monsoon months, when insect populations increase. One reason the virus receives attention during outbreaks is because it can lead to acute encephalitis, a condition in which the brain becomes inflamed. Acute encephalitis is not a disease itself but a medical emergency that can have many causes, including viruses.

The illness can begin like an ordinary fever, but change within hours

One of the biggest challenges with Chandipura virus is that its earliest symptoms resemble many common viral infections.Dr Vivek Jain, Senior Director & Unit Head, Paediatrics, Fortis Hospital, Shalimar Bagh, Delhi, explained, “Chandipura virus infection comes about suddenly. The initial symptoms are high temperature accompanied by a very severe headache, vomiting and extreme fatigue. For children, unusual sleepiness, irritability or confusion might also occur.”These symptoms may not immediately seem unusual, particularly during the rainy season when viral fevers are common. However, Chandipura virus behaves differently.Dr Jain added, “In certain cases, the infection may affect the brain within 24–48 hours and other severe symptoms, such as seizures or loss of consciousness may arise. Because the disease can progress quickly, a child with the fever above normal and showing neurological symptoms needs urgent medical help.”Doctors say parents should not ignore a child who develops high fever along with repeated vomiting, unusual drowsiness, difficulty waking up, confusion, seizures or sudden unconsciousness. These signs require immediate medical attention rather than home treatment.Speed can make a significant difference because supportive intensive care may help reduce complications.

How does Chandipura virus spread, and why are children more vulnerable?

Scientists believe the virus is transmitted mainly through the bite of infected sandflies, tiny insects that are much smaller than mosquitoes. Some evidence also suggests that certain mosquito species may play a role in transmission, although sandflies remain the primary vector identified during Indian outbreaks.Dr Vivek Jain explained, “Transmission of Chandipura virus occurs mainly due to bites of infected sandflies and to some extent, certain species of mosquitoes. Transmission occurs in general but not directly from one person to another.”Health experts say sandflies often breed in cracks and crevices of mud walls, damp surroundings, animal shelters and poorly maintained areas, particularly in rural and semi-urban settings.Children are believed to be at greater risk because they may have had little or no previous exposure to the virus, meaning their immune systems have not developed any protective immunity. In addition, encephalitis caused by viral infections tends to be more severe in younger children.

Prevention is less about medicines and more about avoiding insect bites

There is currently no specific antiviral treatment or widely available vaccine for Chandipura virus. That makes prevention especially important.According to Dr Jain, “Preventive measures include avoiding insect bites by using protective clothing, mosquito nets and insect repellents. Maintenance of cleanliness of surroundings including the elimination of stagnant water and control of breeding of insects is essential. In case of sudden fever in children, early medical assessment is important for timely treatment and help.”

Simple preventive measures include:

Sleeping under insecticide-treated mosquito or sandfly nets, particularly for children.

Dressing children in full-sleeved clothing during evening and night hours.

Using child-safe insect repellents as advised.

Keeping homes and nearby surroundings clean.

Repairing cracks in mud walls where sandflies may breed.

Reducing insect breeding around homes by improving sanitation and environmental hygiene.

Public health officials also encourage communities to report sudden clusters of fever among children so health teams can respond quickly.Medical experts consultedThis article includes expert inputs shared with TOI Health by:Dr Vivek Jain, Senior Director & Unit Head, Paediatrics, Fortis Hospital, Shalimar Bagh, Delhi.Inputs were used to explain what Chandipura virus is, how it spreads, the early warning signs to watch for, why children are more vulnerable, and the steps families can take to prevent infection.

Source link

Chandipura virus: The rare infection linked to recent child deaths explained, from early symptoms and transmission to prevention and treatment When news of children losing their lives to an illness spreads, it naturally raises fear and many questions.
